What weight loss/fitness approach would be best for a woman with sporadic flareups of physical ailments?

I’m a female in my mid forties, and about 60 lbs. overweight. This is weight that I put on quickly over the past year, due to chronic fatigue syndrome, severe depression, and the medications, inactivity and food cravings that go along with those disorders. Now, I am beginning to have breathing difficulties, as well, which my doctor thinks can be greatly reduced or eliminated purely through weight loss. The stage of life I’m in doesn’t help. My metabloism has slowed to a crawl, and everything I’ve tried to do on my own has failed to produce a loss of more than about 11 lbs. Part of the problem is that there are days when my body just isn’t up to moving much more than from the bed to the bathroom. On other days, I can speedily walk three miles around an indoor track with no ill effects. So, I have a serious yo-yo effect going. If I can afford to get into a program with personal training, monitoring and advice, what type of place should I go to considering my medical conditions?
